Fax information required by referring physician:
- DCH patient referral form (script signed by PCP)
- Insurance/Medicaid card (front and back)
- Physician’s notes/diagnosis
- Current labs
- Recent X-rays, CT scans, MRI scans and/or EGGs (Important - Please bring actual films or images)
- Any school diagnostic testing that has been completed
Patient must bring to appointment:
- Patient must be accompanied by parent/guardian (with ID) who knows the history
- ID card
- Immunization card
- Current medications
- New patients need to fill out and bring the questionnaires they received in the mail
All pure ADD, ADHD, and Autism Diagnosis must be referred to Developmental Pediatric Medicine.
The following situations require the PCP call the neurologist in order to communicate the details:
- Urgent referrals (call neurologist on call)
- Neurology second opinions (call neurologist on call)
- Patient no-showed initial consult (call neurologist previously scheduled with)
