Your child’s immunization records
Proof of insurance
Medical records from previous doctors related to the issue for which your child is being referred
Test results related to your child's current issue, such as blood tests, X-rays, CT scans, MRI scans or EEGs. For X-Rays, CT scans and MRI scans, please provide the actual films or discs so our doctors can review them firsthand.
