Knowing exactly what medications a patient is currently taking—even vitamins, eye drops, or over-the-counter remedies—is essential to providing safe and effective care. By answering the questions below, you help our medical team prevent harmful interactions, ensure accurate treatment, and support your recovery every step of the way.

 Does Your Child:

  • Take any prescriptions or medications?
  • Take anything for allergies?
  • Have any emergency medications (epipen, glucagon, seizure meds)?
  • Take anything to help them sleep?
  • Take any vitamins?
  • Take any eye or ear drops?
  • Use any nasal sprays?
  • Use any inhalers or anything for a nebulizer?
  • Take anything for upset stomach, nausea, diarrhea, constipation?
  • Use any topicals such as creams or ointments?

For your child’s safety, different healthcare workers may ask you the same questions more than once. This helps us double-check that we have the correct list of your child’s medicines.

Let us know if you brought any medications with you or have photos of the bottles or labels.
