Referrals with the following diagnosis need to be sent to Corpus Christi for the initial consultation:
- Obesity
- Acanthosis nigricans
- Fatty liver
- Abnormal liver enzymes
- Dyslipidemia
- Hypercholesterolemia
- Hyperinsulinemia
- Metabolic syndrorne or dysmetabolic syndrome
- Abnormal weight gain
- Hyperglycemia
Referring Physicians Must Fax the Following Information:
- DCH patient referral form (script signed by PCP)
- Authorization for Dietary Consult
- Insurance/Medicaid card (front and back)
- Physician’s Notes/diagnosis
- All labs and X-ray results
- Current medications
Patients Must Bring the Following to Appointment:
- Patient must be accompanied by parent/guardian (with ID) who knows the history
- Immunization card
- X-ray film (Bone age - copy or CD)
